My CalvertHealth comes from your CalvertHealth Electronic Health Record to ensure that you have access to the most accurate, up-to-date information possible.

* Please note that the My CalvertHealth Patient Portal uses a separate system and requires different login information than the patient portal service available through your doctor. For any questions regarding your portal through your doctor’s office, please contact their office directly.

My CalvertHealth Patient Portal allows you to:

  • View laboratory and radiology results
  • Find a list of medications and allergies
  • Track visit history
  • Update demographic information
  • Provide access for authorized healthcare proxies
  • Review upcoming appointments scheduled or request a new appointment
  • Make payments
  • View most of your inpatient health information and limited Emergency Room information

How to self-enroll
If you have a valid email address on file with CalvertHealth, you can set up an account now by completing the Enrollment Request form. After completing the Enrollment Request form, you will receive an automated email with a link allowing for you to create login information for your My CalvertHealth Patient Portal account.

To successfully enroll, the same email address that is on file with the health system must be used during the enrollment process. If you do not have an email address on file, you will receive an unsuccessful error message when completing the Enrollment Request form. You will need to visit our Medical Records Department with a valid photo ID and complete a consent form that includes the email you wish to use.

In addition to handling your own care, parents and healthcare proxies with authorized consent will appreciate the ability to access the records of those individuals for whom they manage care. To establish an authorized proxy for a My CalvertHealth Patient Portal account, please contact the patient portal liaison or visit the Health Information Management Department with proper photo identification for all involved parties or a birth certificate for a minor.

My CalvertHealth Portal will only provide health information for an inpatient stay or and emergency room visit. Hospital Records for an inpatient stay or an emergency room visit at CalvertHealth for my child, mother, father, or individual in which you are the primary caregiver or Power of Attorney.
  1. To gain access to medical records for an individual over 18 years of age, the patient must provide the Medical Records Department with a copy of the patient’s photo ID and a proxy formed signed by the patient allowing another individual permission to view medical records.
  2. To gain access to medical records for an individual less than 18 years of age, the parent or guardian must complete a proxy form and provide a photo ID to the medical records department at CalvertHealth. The parent or guardian must be listed in the medical record otherwise permission may not be granted.
My Doctor has asked for medical records from an inpatient stay or ER visit.
Your doctor’s office should call CalvertHealth’s Medical Records Department to request a copy of any needed medical records. Patients may also request records in person with a valid ID and completed consent which can be obtained in medical records. Contact Info: 410-535-8275

Frequently Asked Questions

Some of my information is wrong. How do I correct this?
If you notice an error in your personal information, such as address and phone number, please send us a change request through the portal. The change request can be found by clicking Profile, and then Update Profile.

What if I need help?

How do I communicate my medical concerns?
My CalvertHealth is not used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 911 OR YOUR PHYSICIAN'S OFFICE IMMEDIATELY.

How do I see my test results?
Your test results and reports are both available by clicking on the Health Record button on the portal home page. Once you click on the Health Record button you will be taken to the Health Record page where you will see buttons on the right labeled Results and Reports where you will have access to your Laboratory Results and Other Reports.

How can I request my medical records through the Patient Portal?
In a browser window visit the following URL https://www.calverthealthmedicine.org/Medical-Records.

In this section, page click on the Download Medical Release Form (70k) link. Print out this form, fill it out, attach a photocopy of your photo ID (Driver’s License, State ID, Military ID; or CalvertHealth employee badge) and mail/submit the form as per the instructions on the form.

I don't have an email address, how do I get one?
There are many free email services available to use. Any email service is compatible with My CalvertHealth. If you are looking to create a free email account, some of the more established email providers are:
How can I update my email address in My CalvertHealth?
To change the email address that My CalvertHealth uses for sending activity, password reset, or other communications, please click the Profile button, then Update Profile. Please contact the Patient Portal Liaison at 410.535.8396

What information can I see in My CalvertHealth?
You can see information from your medical record such as laboratory and radiology test results, medications, allergies, immunizations, medical history and current health issues. You may also print a Health Summary for your records.

Is there a fee to use My CalvertHealth?
No. My CalvertHealth is a free service offered to our patients.

How is My CalvertHealth secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal IDs and passwords. My CalvertHealth uses the latest encryption technology. All My CalvertHealth messaging is done while you are securely logged on to our website.

Can I also see my family members’ health records?
Yes, this is called Proxy Access. For a child under 18, this allows a parent or legal guardian to log into their personal My CalvertHealth account, and then view information regarding their child. Per State and Federal guidelines, after the child reaches age 18, parents/guardians will need an additional authorization signed by the child to have access to the child’s medical record information.

Once a child turns 18, a parent can only access to the child’s patient medical record information with the patient’s authorization. It is also possible to request access to another adult’s health record if you help manage that adult’s medical care. If you need additional information than what is available through My CalvertHealth, please contact the Health Information Management department at 410.535.8275.

How can I signup?
If you know your Medical Record Number and you have provided your email address when you last registered, you can go to CalvertHealthMedicine.org and click on the Set up an Account button.
