Diabetes Self-Management Education and Support (DSMES)

  • Helps people with either Type 1, Type 2, or Gestational Diabetes learn practical skills to fit diabetes care into your life and find what works for you.
  • Provides personalized services to help you manage diabetes.

If you’ve been diagnosed with breast cancer, your cancer care team will discuss your treatment options with you. It’s important that you think carefully about each of your choices. Weigh the benefits of each treatment option against the possible risks and side effects.

DSMES Individual

You will meet one-on-one with a diabetes educator. They will help you learn about diabetes and make a plan to manage food, blood sugar, and more.” Primary care provider referral is required.

DSMES Group

  • DSMES Wellness Workshop
    • The Diabetes Support Group meets in-person at the Prince Frederick Public Library on the 2nd Tuesday of the month at noon-1 p.m. Register online.
    • Join our subject matter experts with a Q&A at the end of each meeting.

DSMES Series

A certified program by the American Diabetes Association. This three-part class is offered in-person and is taught by a Registered Dietitian and a Certified Diabetes Care and Education Specialist Registered Nurses. Learn about healthy eating choices, checking blood sugar levels, fitness, how diabetes affects your body, and more. Classes are available day and evening.

Medical Nutrition Therapy (MNT)

  • MNT is a one-on-one visit with a Registered Dietitian.
  • You will learn how food can help manage health problems like heart disease, weight concerns, stomach issues, and food allergies.
  • The dietitian will help with meal planning, healthy food choices, and weight goals. Provider referral required.
